New Patient Registration

Our practice is open to new patients who live within our practice area. Please telephone our practice for further information.

Registration is now paper-free

 

Please complete a registration form and new patient questionnaire - the download links are at the bottom of this page. You may need to download Adobe Acrobat Reader to edit and save your form.

When you have completed your forms, please email them to our practice:  prescription.s77248@nhslothian.scot.nhs.uk 

After you have emailed your registration form you will receive an autoreply within half an hour letting us know that we have received your email. If you do not receive an autoreply then please telephone us on 0131 322 9333.

If you do not have an email address or have any problems opening the registration forms, please telephone the practice. We can post paper forms to you or arrange for you to collect paper forms from our reception desk. 

Once we have received your registration forms we will fully register you within a week. Medical treatment is available from the date of registration. If we are unable to register you for any reason (for example, if you do not live in our practice area) then we will let you know. 

Patients should note that the law has recently changed in Scotland regarding Organ Donation and you are no longer asked about your wishes on your registration form. Please visit https://www.organdonationscotland.org/  or telephone 0300 123 23 23 for more information on opting in or out of organ donation.
